COURSE REGISTRATION, CANCELLATION & REFUND POLICY

This Registration, Cancellation, and Refund Policy (“Policy”) constitutes a legally binding agreement between the participant (“Participant”) and SI Institute (“Institute”). By registering for any course, the Participant acknowledges that they have read, understood, and agreed to the terms below.

1. Non-Refundable Tuition

All course tuition payments are non-refundable, except as expressly stated herein. The Institute incurs substantial upfront expenses including, but not limited to:

· Faculty compensation and clinical staff allocation.

· Clinical supplies, sterile materials, and specialized equipment.

· Patient coordination, screening, and surgical scheduling.

· Facility preparation and administrative staffing.

Accordingly, tuition payments secure a reserved seat in a limited-capacity clinical course and are not contingent upon attendance.

2. Participant Cancellation

A. Cancellation More Than Thirty (30) Days Before Course Start Date: If a Participant provides written notice of cancellation more than 30 calendar days prior to the start date:

· Tuition is non-refundable.

· The Institute may, at its sole discretion, allow a transfer of tuition to a future course date (valid for 12 months).

· Approved transfers are subject to an administrative fee of $750.00.

B. Cancellation Within Thirty (30) Days of Course Start: If cancellation occurs fewer than 30 calendar days before the course start date, no refund will be issued, and tuition is forfeited in full.

3. No-Show / Same-Day Cancellation / Partial Attendance

The full tuition amount is forfeited if a Participant fails to attend, arrives late, or leaves early. The Institute may, at its sole discretion, allow a transfer to a future date for a rescheduling fee of $3,500.00.

4. Travel Disruptions and Personal Circumstances

The Institute is not responsible for airline delays, weather events, illness, or emergencies. Participants are strongly encouraged to purchase travel and event insurance.

5. Course Cancellation by the Institute

If the Institute cancels a course due to enrollment, faculty emergency, or patient availability, the Participant may choose:

· A full tuition refund, OR Transfer of tuition to a future course date.

· The Institute is not responsible for travel, lodging, or incidental expenses.

6. No Chargebacks / Payment Disputes

Participant agrees not to initiate a credit card chargeback. In the event of a dispute, Participant agrees to reimburse the Institute for all associated fees and reasonable attorney’s fees.

7. Arizona Volunteer License Requirement

1. Licensure Requirement: Out-of-state dentists must obtain appropriate Arizona authorization.

2. No Facilitation: SI Institute does not process or manage licensure applications. This process can take 6–8 weeks.

3. Proof Required: Proof of valid Arizona licensure must be provided prior to clinical participation.

4. Failure to Obtain License: If licensure is not secured, Participant may attend lecture portions only; no refund or credit shall be issued.

5. Rescheduling Fee: Rescheduling due to licensure failure is subject to a $1,500.00 fee.

Live Patient & Clinical Procedure Disclaimer

1. Patient Cancellations: Patients may cancel or become medically ineligible at any time.

2. Clinical Limitations: Procedures may be modified or canceled due to intraoperative findings, anatomical limitations, or patient safety.

3. No Guarantee of Numbers: The Institute does not guarantee a specific number of implants. Projection numbers are estimates, not contractual guarantees.

4. Educational Nature: Tuition is paid for instruction and training, not for a guaranteed quantity of surgical procedures. No refunds are issued for reduced clinical opportunities.

9. Media Release & Publicity Authorization

Participant grants Institute the irrevocable right to record their image, voice, and likeness for marketing, social media, and educational materials. Participant waives the right to inspect final materials and acknowledges no compensation will be provided.

10. Governing Law & Entire Agreement

This Policy is governed by the laws of the State of Arizona. This constitutes the entire agreement regarding cancellations and refunds.
